Position Description:  JOB SUMMARY

Provides individual, family, and group therapy to youth and families. Provides crisis intervention, clinical assessment, direct service, and referrals.

MINIMUM QUALIFICATIONS

Grade 26 : Current license in the State of Utah as a Certified Social Worker (CSW), Certified Professional Counselor Intern (CPCI), Associate Professional Counselor (APC), Licensed Associate Marriage and Family Therapist (LAMFT), or a Marriage and Family Therapist Intern (MFTI).

Grade 28:  Current license in the State of Utah as a Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or a Licensed Marriage and Family Therapist (LMFT).

Due to the nature of this position, passing a background check is required.

ESSENTIAL FUNCTIONS 

• Performs clinical assessments, develops crisis and treatment plans based on the assessment, tracks and modifies the plan as required.
• Provides direct clinical services including crisis intervention, short term individual, family and group counseling.
• Provides case management which may include coordinating and linking families to community resources, consulting with other providers, and making referrals as needed. 
• Completes case documentation and all required reports in the appropriate format and in a timely manner.
• Performs other related duties as assigned, including but not limited to, supervising interns, holiday clinical coverage, attendance at meetings, and serving on committees.
• Complies with codes of professional ethics; strictly follows agency policies, procedures and code of conduct.

Renewal of Federal Block Grant Sought: Comment Period now Open

WHO/WHAT:  The Utah Department of Health (UDOH) is asking for public comment on the renewal of its federal Preventive Health and Health Services Block Grant for federal fiscal year 2012, during a public hearing. 

     

WHEN:  Thursday, February 9, 2012 from 5:30 – 6:30 p.m.

WHERE: State Capitol Building, Room 130, Multipurpose Room 250 N. State Street, Salt Lake City, UT 

WHY:    The Department is seeking public input on how $681,316 in grant funds, could be spent in Utah to meet select federal Healthy People 2020 public health objectives. 

Currently, the block grant funds are used by the state and local health departments for preventive health services, including obesity prevention and injury prevention. A federally-mandated set-aside in the block grant funds rape crisis and prevention services.  Due to a nearly $300,000 reduction in grant funds in federal fiscal year 12, support for community assessment and environmental health efforts was reduced.

OHD & University of Michigan-Flint partner to bring Cultural Competence Training for Healthcare Providers in March

The Utah Department of Health (UDOH) Office of Health Disparities Reduction (OHD) in partnership with the University of Michigan-Flint (UMF) will offer an in-person training especially designed for health care providers (emphasis on nurses) to enhance cultural competencies and develop skills in meeting the needs of diverse patients, groups and communities. 

It is well known that a culturally competent health care workforce is needed to promote healthy lifestyles and reduce health disparities.  It is crucial for health care providers to communicate with their patients and understand what constitutes illness within the framework of their cultures in order to mutually decide upon care that is evidence based and culturally appropriate.

Local Grocery Stores Present New Eating Smart with Diabetes Store Tours

Salt Lake City, UT (February 25, 2012) – A new diabetes education and nutrition program is being offer for FREE to interested patients and caregivers at select Associated Food Stores along the Wasatch Front.

These in-store events provide people impacted by diabetes with a unique opportunity to learn how to better manage the disease and how to shop for menu choices to improve health.

Each 90-minute class is led by a specially trained Diabetes Care Pharmacists and a Registered Dietician. The class begins with a pharmacist-led presentation regarding diabetes, proper testing, and medication adherence. The Registered Dietician teaches food options, portion sizes, and nutrition label decoding. The majority of the class is spent walking through the store.

According the Registered Dietician, Harmony Mitchell, “This class is different than your typical diabetes education class because it is interactive. We teach in a hands-on environment. We have the advantage of offering a grocery store shipping experience to reinforce nutrition training.”
