OHD is requesting your help

Do you live in Glendale or South Salt Lake?
The Utah Department of Health Office of Health Disparities is conducting a survey to gain a better understanding of how residents access health care and oral health services in certain areas.  We also want to learn more about living conditions facing those residents. The goal is to ensure that there are enough members of racial and ethnic minorities in the sample so that more reliable estimates of health indicators can be reported, and more precise measures can be taken to address geographic disparities. Participants will be entered into a drawing for $50 grocery gift cards.
