 Help People Learn to Manage Chronic Conditions

The Active Aging Program of Salt Lake County Aging & Adult Services is recruiting volunteers who are interested in helping teach persons with chronic health conditions about ways to manage their symptoms. Living Well with Chronic Conditions Self-Management Program is a workshop given two and a half hours, once a week, for six weeks. People with different chronic health problems attend together. Workshops are facilitated by two trained leaders, on or both of whome are non-health professionals with a chronic disease themselves. Workshops are free. Peer Leaders will need to attend the program training on June 12-14 from 9 am to 4:30 pm, at the University of Utah Farmington Health Center.
