State Primary Care Grant Program Application for FY 2020

The Office of Primary Care and Rural Health, Utah Department of Health, invites qualified agencies to submit an application for the State Primary Care Grant Program funding. The State Primary Care Grant Program supports access to health care by assisting public entities and community-based organizations with the cost of providing primary health care services to medically underserved populations. The grant period will cover July 1, 2019, through June 30, 2020.

As always, awarding funds through this program is contingent on the amount of funds appropriated for this purpose by the state legislature. Please be mindful of any changes to the guidance, application and associated attachments from previous years.

The grant application is open to all primary medical care, dental care, and mental health care sites. Non- profit organizations will need to understand and agree to the checklist for non-profit entity grant contracts.

Only one application is allowed per site. Quality and attention to detail are important to the success of your application. Please be aware that the State Primary Care Grant Program is a competitive program and an Application to the Program does not guarantee an award or future funding.

Applicants should be aware that it is the policy of the Utah Department of Health that agencies awarded under the State Primary Care Grants Program:

  • Must ensure that continuity of service is maintained for the full duration of the grant period
  • Must use SPCGP funding to provide primary care, dental care, and/or mental health care services for the full duration of the grant period; and
  • Must assure that SPCGP funding is used to provide these health care services to legal residents of the State of Utah (as defined in the application).
A complete, original, electronic application must be submitted to UDOH Office of Primary Care by Monday, May 13, 2019, 5:00 p.m. M.S.T.

Applications must be submitted by email to opcrh@utah.gov. (Hand delivered, faxed and mailed copies will NOT be accepted.)
